Health technology assessment od digital diabetes therapeutics

The prevalence of Type 2 diabetes mellitus is increasing all around the world. This chronic disease leads to a decrease in quality of life and brings a significant financial burden to the national health systems. While diabetes not being completely reversible, it can be well controlled by introducing and maintaining healthy living habits. Nowadays, such intervention can be deliv- ered through digital devices with less medical staff time needed. This thesis investigates the cost-effectiveness of the lifestyle intervention delivered by a mobile application on a German study sample of 42 patients. The analysis is performed using a discrete-time Markov chain for the different lifetimes of the model. The robustness of the results is checked using both deterministic and probabilistic sensitivity analysis. Results suggest that digital intervention can be considered cost-effective in both the short and long-term compared to the current standards of care. 1

Economic aspects of screening for thyroid disease in pregnancy and in women with fertility disorders

The incidence of thyroid diseases in pregnancy in the Czech Republic reaches 10- 15%. Emphasis on early diagnosis and treatment is laid not only during pregnancy but also in the time preceding conception due to the impact of thyroid diseases on fertility, the course of pregnancy, birth and fetal development. The aim of the dissertation was to assess the effectiveness and economical aspects of screening for thyroid disease in pregnancy and in women with fertility disorders in the conditions of the Czech Republic. The dissertation consists of four published studies. The first study is a prospective cross-sectional study of 200 positively screened pregnant women. In the study we come to conclusion that pregnant women who are at high- and low-risk for thyroid disease have similar clinical and laboratory characteristics and screening, currently focused only on risk groups, is ineffective. The second study of 5 223 pregnant women is a case-control study. We find that the age of women over 30 cannot be regarded as a risk factor for thyroid disease in pregnancy, although addition this age criterion to the case-finding screening strategy improve its efficiency and ATA (American Thyroid Association) include it in their guideline 2011. Economic evaluation of surgery compared with pharmacological treatment for patients diagnosed with gonarthrosis

The main purpose of my diploma thesis is the application of economic evaluation principles; costs quantification and the consequences quantification of two different approaches in the gonarthrosis therapy from the perspective of Health Insurance Company and from the perspective of the patient; furthermore the evaluation of the effectiveness and utility ratio and the final determination for more convenient option. Considered alternatives are the hyaluronic acid injection and the total knee arthroplasty realization. Effects for the expenditures at cost-effectiveness analysis are measured by WOMAC Index. Effects for the expenditures at cost-utility analysis are measured by HUI2 score system. The research includes two sections: (1) the case study of two patients and (2) the clinical trials and nationwide Health Insurance Company´s data analysis. Particularly because of the brief observation time period, the pharmacotherapy is quantified as a more convenient alternative.

Economic analysis of energy storage devices contained in EFSI 2015-2017

As a result of support of renewable energy sources in Europe the fluctuations in electricity production occurs and threaten the stability of the electrification network. For this reason, we can monitor growing demand for technology solutions that are able to effectively eliminate these fluctuations. Devices for storing electrical energy appears suitable. This thesis deals with the economic analysis of the projected facilities in the Czech Republic that have applied for support from the European Fund for Strategic Investment (EFSI), compared to other model devices. As part of the work there was used method of cost-effectiveness analysis to evaluate various technology solutions based on the ability to provide ancillary services. Consequently on the results of thesis we can say the most cost-effective project is under current conditions, construction equipment storage technology using compressed air CAES located at Dolní Rožinka included in EFSI 2015-2017. The thesis is concentrated on the analysis of the decision - making process. Concretely in the case of building-up of R35 highway. The analysis describes landscape planing process, strategic environmental assessment process, environmental impact assessment process, planing inquiry and buliding control. I'm trying to find optimum alternate of building-up of the highway as a combination of economic,environmental,technological and social point of view.
